The first Victorian Australian primary care collaborative's meeting of the second phase has concluded. The meeting was run very professionally and provided significant stimulus and ideas for participants to proceed with PDSA cycles over the coming months.
The roles and tasks required of participants include, baseline data collection using available software Clinical Assessment Tools [CAT] which has been facilitated in our case by our liaison officer with the Monsh Division of Gen. Practice. Ongoing data collection is a large part of the role. We will now be collecting results of surveys of patient satisfaction with regard to accessibility and timing of appointments.
We'll also begin collecting data regarding accessibility. This will include documenting every request for an appointment and the outcome of the request [whether the request was fulfilled, unable to be filled due to lack of appointment time or practice policy].
We will create a diabetes and coronary heart disease patient register. These registers will be maintained by staff within our practice and utilized to facilitate enhancing primary preventive care for the patients involved.
Software to the rescue
After the first night of the collaborative meetings I sat down and pondered one particular presentation given by a practice nurse. The nurse had spent two weeks creating a register of diabetes patients. The significant investment in time was required due to the poor export facility available within the medical director diabetes register. The nurses requirements included the ability to export demographics and information regarding
- latest hemoglobin A-1 C,blood pressure, microalbumin, renal function,
- podiatry, diabetic educator, diabetes SIP, Team Care Arrangement,
- medications-particularly aspirin and statins
It was obvious during the presentation that most of the participants did not have the time and resources to dedicate to the creation of any diabetes register as outlined. It was also obvious that this was something which a software tool (such as the DCP) was eminently suitable to provide this function. Therefore I sat down on the night after the first presentation and added this function to the DCP and today I also created a demo vid, posting it on this website.
