Re: Digital Microscope ($70) Replaces my Dermatoscope plus Canon EOS w Macro Lens & (~$4000)
I have no obtained a dig microscope following reading this. Works very well. Still takes some time to import pictures into software.
Re: Digital Microscope ($70) Replaces my Dermatoscope plus Canon EOS w Macro Lens & (~$4000)
We have now bought one to try as well. All docs in clinic have decided to get one after demoing it.
Re: Getting Growled at!
Hi Phil
DCP needs to be updated to the latest version for Growl. I have tested the MD3 and BP versions.

Other than that - I’m not sure what may be preventing Growl registering the DCP.
Perhaps firewall - the Growl app sends /receives messages via TCP, but I had no problem with growl and Norton Firewall . As it is on the local machine it should not be a problem.
Cheers
Re: Getting Growled at!
When Growl is running it doesn't notice the DCP starts up i.e. DCP is not registered with Growl. What may be blocking it?
Re: DCP Subscriptions - Soon available
Any update on how we can have non-doctors writing/contributing to care plans. Especially the AUSDRISK assessments!! At the moment, the nurses are doing AUSDRISK, but it is not being recorded in Doctor's Control Panel, and so the risk of unnecessary repetition is high.

Re: DCP Subscriptions - Soon available
please i would like to get more information about the subscription, how much it costs per year and how can i subscribe
thanks
UPDATE 2010
The k550 was retired.
I have now changed all printers over to HP 5400 dt models at the surgery. All have CISS. No problems with any of them.I was able to hunt around and find the superceded K5400 models at a rediculously CHEAP price $70-$100 at officeworks (scrounged all the stores in Victoria). I would have bought 10 at that price. Even with $130 for each CISS it is cheap.Total running cost for surgery is about $100 per year. Down from $1000+. Plus no inconvenience of replacing cartridges. I have two in my office plus a small epson for envelopes.

We are implementing a simple PDSA for weight, height and waist measurement.

This utilises features of the PIP prompter program.

Here are the statistics as  collected at baseline

Checking [HEIGHT, WEIGHT, WAIST]

Total patients checked = 4652

Here is the PDSA outline and I have posted a word document as well.

Background

Red Book Guidelines for prevention of obesity and concomitant morbidity risks.

What to measure

1.       Height

2.       Weight

3.       Waist circumference

4.       Pubertal stage in children

When to measure

Risk Groups

Paediatric (<18yo)

Adult(>=18yo)

All Australians

Aboriginal and Torres Strait Islander peoples and those from Pacific islands

6 months

12 months

Middle Eastern and Mediterranean backgrounds

Existing diabetes or cardiovascular disease, stroke, gout, liver or gallbladder disease

Intervention

Preventive counselling and advice for physical activity and nutrition advice. Details on overweight and obesity management can be found in NHMRC Overweight and obesity in children and adolescents – a guide for general practitioners

www.health.gov.au/internet/wcms/Publishing.nsf/Content/obesityguidelines-guidelines-gp_guide.htm/$FILE/children_gp.pdf

PLAN

Aims

Implement red book guidelines for height, weight and waist measurement.

Implement these as patients present during routine practice.

How

Implement these via software assisted data collection sheets used prior to consultation and via prompts during consultation.  Data is recorded into our practice software (Medical Director).

Identify Outcome/Change

Be able to record statistics regarding current recording of data for each of the different groups and intervals as specified in the Red Book Guidelines.

Limitations in design

Currently able to easily integrate within software parameters

1.       Age

2.       ATSI status

3.       existing obesity

4.       history of cardiovascular disease, stroke and chronic liver disease

Not able to distinguish

1.       Middle Eastern and Mediterranean backgrounds

2.       history of gout or gallbladder disease

Difficulties with staff implementing waist measurements pre-consultation meant waist only recorded if measured during consultation.

Not able to record pediatric pubertal stage within software.

Changes expected

Increased recording of Red Book guideline data

Increased utilization of features of medical electronic record for recording data

Integrate successful techniques for recording weight, height and waist measurement into routine medical practice.

Increase interventions for obese patients.

Cycle 2

Implementation of Weight and Height following Red Book Guidelines

Method

Install PIP Check Prompter on all participating doctors’ workstations.

Run application during consultation to receive prompts for preventive care items height, weight and waist measurement.

Print daily data collection sheets for staff at front desk to implement pre consultation measurements.

Record measurements in MDW during consultation and implement interventions if deemed appropriate.

Cycle 3

Implementation of Weight and Height following Red Book Guidelines

Ongoing refinement of Processes

Refine and enhance the pre consultation measurement of height, weight and waist to facilitate speed, usability for patients and staff.  Try different methods.

Assessment of Change

Print statistics from Check Prompter and monthly throughout the PDSA cycles.

Observe for patient subgroups (Rules in PIP Check Prompter) which tend to be overlooked and make efforts to improve recording as per Red Book Guidelines.

Cycle 4

Implementation of Weight and Height following Red Book Guidelines

Assess Implementation of Red Book Guidelines for height, weight and waist within the practice over the course of the PDSA.

Areas for improvement.

Pre Consultation.

During Consultation.

Applicability to Other RBG Data

Other Red Book Guideline data collection that can be implemented / enhanced used methods discovered in this PDSA.
